SUBJECT : EXTENSION OF THE SUBMISSION OF
ONLINE DORMITORY APPLICATION

DATE : 17 May 2016

As per meeting of the Dormitory Admissions Committee today, 17 May 2016, and due to the requests we received from some students, the DAC decided to extend the submission of the online dormitory application until 20 May 2016, which will coincide with the deadline of the submission of required documents.

Please be guided accordingly.

Application for AY 2016-2017

a. For NEW Applicants
Students who are FIRST-TIME applicants in the residence hall

*Printed Application Form
*1 recent 2x2 photo
*1 long brown envelope with your name and student number written legibly at the back
*Proof of Income
- Income Tax Return of the graduate student or his/her parents/spouse or
- Proof of Indigency or
- Contract of Parents (for OFW)
* Copy of Grades (True Copy of Grades or CRS printout with certification)- for upperclassmen


b. For Reapplication (Returning)
Students who stayed in the residence hall before AY 2015-2016

* Printed Application Form
* 1 recent 2x2 photo
* 1 long brown envelope with your name and student number written legibly at the back
* Proof of Income
- Income Tax Return of the graduate student or his/her parents/spouse or
- Proof of Indigency or
- Contract of Parents (for OFW)
* Copy of Grades (True Copy of Grades or CRS printout with certification)
* Dormitory Clearance (from previous residence hall)


c. For Readmission
Students who are currently staying/stayed in the residence hall during AY 2015-2016

* Printed Application Form, if there are updates in your profile
* Proof of Income
- Income Tax Return of the graduate student or his/her parents/spouse or
- Proof of Indigency or
- Contract of Parents (for OFW)
* Copy of Grades (True Copy of Grades or CRS printout with certification)
* Dormitory Clearance (from current residence hall)

25 April 2016, 12nn – 20 May 2016, 5PM – Submission of Required Documents at the Office of Student Housing, Acacia Residence Hall

23 May 2016 – Release of Results for Midyear Application (including new applicants)

3 June 2016- Deadline for Submission of Dormitory Clearance

10 June 2016 – Deadline for Submission of Copy of Grades (True Copy of Grades or CRS printout with certification)

20 June 2016 – Release of Results for Academic Year 2016-2017

1. The online dormitory application for MidSemester AY 2015-2016 and 1st Semester AY 2016-2017 will open on Tuesday, 12 April 2016, 8 AM until 16 May 2016, 5PM.
2. UP Diliman students including incoming freshmen who have CRS account will be able to fill up the online application at http://dormapplication.upd.edu.ph. For the upperclassmen, kindly use your DILNET account.
3. For those students who cannot log-in, kindly email to dormappsupport@list.upd.edu.ph
the following: Full Name and Student number. Kindly log-in again after 24 hours.
4. For MidYear AY 2015-2016, dormitory slots maybe limited due to the maintenance schedule of the residence halls.
5. The dormitory application team shall accept the required documents for residence hall application for 1st Semester, AY 2016-2017 beginning Monday, 25 April 2016, 12nn at the Acacia Residence Hall.
